Hi, Chris. I do not have such report. However, you may be interested in the following guidelines given in Spanish by Julia Espinosa Fajardo in her publication on gender-sensitive evaluation of development programs (2010) about questions to be considered in the evaluation design. I hope it helps (sorry, pls. use Google translator):

¿Quién es la población beneficiaria de la intervención? ¿Mujeres, hombres, ambos? ¿Quiénes se benefician y quiénes salen perjudicados? ¿Se cuenta con mujeres y hombres en la definición del problema y en la propuesta de solución? ¿Se identifica la desigualdad de género como un problema central en la acción de desarrollo? ¿Cómo se define el concepto de género, de relaciones de género y cómo se plantea su relación con la intervención? ¿Existe reconocimiento de la existencia de problemas específicos de mujeres y hombres que son resultado de las relaciones de género existentes? ¿Se plantea una estrategia dentro de la acción de desarrollo para impulsar la igualdad de género? ¿Se identifican objetivos referidos a cambios en las relaciones de género? ¿Y objetivos dirigidos a actuar sobre problemas específicos de mujeres y hombres? ¿Qué mecanismos se adoptan para fortalecer la participación de las mujeres en la intervención teniendo en cuenta su tradicional localización en el ámbito doméstico y su posición subordinada? ¿Se consideran los diferentes roles – reproductivo, productivo y comunitario – de mujeres y hombres? ¿Se valora el diferente grado de poder de mujeres y hombres en la toma de decisiones? ¿Se impulsa la participación equitativa de mujeres y hombres en los espacios públicos y privados? ¿Se impulsa su empoderamiento de las mujeres? ¿Se apuesta por el desarrollo de las capacidades de las mujeres para hacerse cargo de sus propias vidas y actuar colectivamente para solucionar problemas? ¿Se fomenta el control de su propio cuerpo por parte de las mujeres así como el respeto por parte de los hombres? ¿La intervención responde a las necesidades prácticas de género, a las necesidades estratégicas de género o a ambas? ¿Se tiene en consideración los distintos usos del tiempo de hombres y mujeres? ¿Se tienen en cuenta el desigual acceso a recursos económicos, políticos y culturales por parte de mujeres y hombres? ¿Y el desigual control sobre los mismos? ¿Se atiende a la relación de la desigualdad de género con otros tipos de desigualdades vinculadas a la etnia, la sexualidad, la casta, etc.?

Dear Chris, although there are not "universal" questions to be used to evaluate those issues, as each program would need to design their own gender-related and social inclusion questions, I personally find the following set of basic questions very useful. They were originally prepared for an evaluation of a USDA-funded educational program: 

Gender– cross-cutting theme to assess gender parity and gender dynamics such as female participation and decision-making power.

  • To what extent are project activities reaching female and male beneficiaries equally?
  • What differential outcomes and impacts, if any, are project activities having on women and girls? Men and boys?
  • To what extent is the project actively addressing gender-specific barriers and constraints to education and community engagement?
  • What specific actions and interventions has the program fostered to promote gender equality, particularly in marginalized schools (e.g. schools that have low gender parity and other gender biases)?
